介護老人福祉施設においても,施設の個室に閉じこもる高齢者が増えてきている.
本研究の目的は,介護老人福祉施設で自分から人との交流をもとうとしない高齢者の思いを明らかにすることである.
介護老人福祉施設入所高齢者10人に半構成的面接を行った.対象者は,男性3人および女性7人で,平均年齢は76.8±7.9歳,入居期間は6カ月から14年であった.所属大学の倫理委員会審査で承認を得て行った.その結果,【ここの暮らしになじめない】,【周りに人がいるけれどさみしい】,【ここではしたいことがない】,【自分から人とかかわりたくない】,【体の調子が悪いので何かをしようと思わない】【気兼ねせず好きなようにしたい】,【人に迷惑をかけたくない】という7つのカテゴリーが抽出された.人や環境に対してなじめないので,自分から人とかかわり,したいことを見つけようとすることができず,体調不良や人に迷惑をかけたくないという思いが,新しい環境に働きかけることを消極的にさせ,さみしい思いで過ごしている高齢者の姿が浮かび上がってきた.
今後に向けては,高齢者が新しい環境になじめるような場づくりや,入居当初から関係形成を計画的に進めることが必要と思われた.また,活動が消極的にならないように体調管理が重要であると考えられた.
Description Alternative
An increasing number of elderly people are confining themselves to their rooms even at nursing home. The present study aimed to clarify the feelings of nursing home residents who do not proactively attempt to interact with others.
Semi-structured interviews were conducted on 10 nursing home residents (3 men and 7 women with an average age of 76.8 years, standard deviation of 7.9 years ; length of residency,6 months‐14 years) following approval from the ethical review board of our university. We categorized the narratives of the subjects in the verbatim record. The following 7 categories were extracted from interview transcripts : ‘I am unable to adjust to living here’ ; ‘Despite there being people around me, I am lonely’; “There is nothing I want to do here’ ; ‘I don’t want to approach others’ ; ‘I can’t think of anything to do due to my poor health’ ; ‘I want to do as I like without hesitation’ ; and ‘I don’t want to bother others’. These categories depict nursing home residents as living in loneliness due to passivity in the environment promoted by feelings of not wanting to bother others, poor health and an inability to approach others or attempt to find desirable things to do due to failure to adjust to the people or environment.
The present findings indicate the necessity of creating an atmosphere in which elderly residents can adjust to the environment and can systematically begin to form relationships from the start of residency. Management of physical health is also required to prevent residents from becoming passive.
