乳幼児健診に従事する保健師のストレッサー構造に関する研究

目的：乳幼児健診に従事する保健師のストレッサーの構造について検討することを目的とした．
方法：予備調査として５年以上乳幼児健診に従事した経験のある保健師１１名の面接内容と先行研究により乳幼児健診におけるストレッサー項目を抽出し，その項目から乳幼児健診に従事する保健師のストレッサー質問紙を作成した．４５０名の保健師に自記式質問紙調査を行い，因子分析を用いて分析した．本調査では，予備調査で作成した質問紙を用いて４８９名を対象に自記式質問紙調査を行った．尺度の信頼性に関してはCronbach の▯係数を算出し，妥当性に関してはMBI日本版とGHQ-１２との関連性を検討した．
結果：乳幼児健診に従事する保健師のストレッサー質問紙調査の因子分析を用いて分析をした結果，４因子が抽出された．第Ⅰ因子は「保護者の対応」とする７項目，第Ⅱ因子は「対人関係」とする３項目，第Ⅲ因子は「職務内容」とする５項目，第Ⅳ因子は「能力」とする３項目の全１８項目が抽出された．Cronbach の▯係数は「保護者の対応」は▯＝０．８５，「職場の対人関係」は▯＝０．８６，「職務内容」は▯＝０．７４，「能力」は▯＝０．７７であった．尺度全体では▯＝０．８７であった．いずれも十分な内的整合性が明らかになった．MBI日本版とGHQ-１２との関連性を分析した結果，有力な相関が認められた．
考察・結論：乳幼児健診に従事する保健師のストレッサーの構造は「保護者の対応」，「対人関係」，「職務内容」，「能力」であった．そして本研究で作成した乳幼児健診ストレッサー質問紙は，信頼性，妥当性が確認され，乳幼児健診に従事する保健師がどの程度のストレスを抱えているのかを簡便に測定でき，保健師のメンタルヘルスに寄与するものであると考える．

Purpose：Through a survey questionnaire, this study investigates the structure of the stressor for public health nurses engaged in medical checkups for infants. Methods : As a preliminary study, stressor items in medical checkups for infants were extracted using previous study results and data from interviews involving １１ public health nurses engaged in medical checkups for infants for more than ５ years, and the author filled up a self-administered questionnaire survey to ４５０community health nurses and analyzed it using a factor analysis. In the main study, a self-administered questionnaire survey was conducted involving ４８９public health nurses using a questionnaire prepared in the preliminary study. Cronbach’s ▯ was calculated to assess questionnaire reliability, and the relationship between the Japanese version of the Maslach Burnout Inventory（MBI）and １２-item General Health Questionnaire （GHQ-１２）was examined to assess validity. Results：Four factors were extracted as the results of factor analysis of the stressor inventory survey of a community health nurse engaging in medical checkups for infants. Seven items regarding “correspondence of guardians”，３ items for “interpersonal relationships”，５ items for “job contents”, and １８items for “ability” were extracted as Ist, IInd, IIIrd, and IVth factors, respectively. Cronbach’s ▯ was０．８５，０．８６， ０．７４，０．７７，and ０．８７for “correspondence of guardians”， “interpersonal relationships at workplace”， “job contents”， “ability”, and all items, respectively, showing sufficient internal consistency. Discussion and Conclusion : The structure of the stressor of the community health nurse who engaged in medical checkups for infants was “correspondence of guardians ”, “interpersonal relationships ”, “job contents ”, “ability”. And, as for the medical checkups for infants’ stressor questionnaire survey reliability, and validity were confirmed in the present study. This stressor questionnaire can simply measure stress levels in public health nurses engaged in medical checkups for infants, and contribute to improving their mental health.
