保健師が導く健康づくり（労働者の行動変容につながる保健指導） : 保健師が関わった糖尿病地域連携

Through analyzing results of specific health checkups started from２００８for persons insured by National Health Insurance in Mima city using overview system for diabetes, many untreated persons requiring medical examinations were detected, suggesting that large parts of targets of the specific counseling guidance need to be managed by the regional healthcare／medical treatment corporation especially between medical institutions and public health nurses. Based on this results, passing for regional healthcare／medical treatment corporation between public health nurses／registered dietitians and certified physicians were started for untreated persons requiring medical examinations from August,２００９.
This approach made certain of medical treatment of targeted persons, resulting into their earlier diagnosis／treatment as well as improved life style This approach also help to build good relationship among members of regional healthcare／medical treatment corporation system including physicians.
Further work using this approach may contribute to the secondary prevention of diabetes through increasing checkup rate, preventing cessation of treatment, decreasing the number of untreated persons, and regional health promotion.
